A massive study examining dozens of side effects attributed to popular cholesterol-lowering drugs has found that only four are actually caused by the treatment.

People aged fifty and over should check their medicine cabinet: you will likely find a statin there. These drugs are one of the great success stories of modern medicine. Designed to prevent heart disease and stroke by reducing LDL levels (the "bad" cholesterol), they are taken daily by about 200 million people worldwide. Atorvastatin (Lipitor) has been one of the best-selling drugs in England in recent years. However, statins are often criticized due to attributed side effects—ranging from muscle pain to memory impairment—which deter many from taking them. In Israel, the consumer leaflet for atorvastatin lists about sixty undesirable effects. A new article in The Lancet reveals that, alongside two effects proven in previous studies, only four additional effects out of 66 examined are actually caused by these drugs.

Strict Screening

This project is backed by the CTT group, an international research network at the University of Oxford that aggregates data from major clinical trials in the field of cholesterol. To verify the link between side effects and statins, researchers analyzed data from 19 clinical trials involving about 124,000 participants. Only studies lasting at least two years with over a thousand participants were included. All were conducted using a double-blind method, reducing reporting bias—the tendency of patients to report symptoms due to expectations or to mistakenly attribute them to medication. Four additional studies compared high and low doses of statins, involving about 30,000 participants.

Because different medical terms were used over thirty years of research, scientists standardized millions of records, merging similar terms (e.g., "nausea" and "vomiting") to define 66 distinct effects.

Four Out of 66

Analyzing such a large volume of data carries the risk of false-positive results due to random statistical fluctuations. To avoid this, researchers used a rigorous statistical model with a strict screening threshold. The result was unequivocal: a real link to statins was confirmed for only four effects: increased liver enzymes (transaminases), other abnormalities in liver function tests, edema, and changes in urine composition.

The analysis showed that the risk of liver function abnormalities is dose-dependent and most pronounced in patients taking atorvastatin at the maximum dose of 80 mg. In this group, the risk of transaminase abnormalities was twice as high, and the risk of other liver function test abnormalities was three times higher than with a placebo.

Two known side effects—type 2 diabetes and muscle pain—were not included in this study, as their link to statins is already well-established in previous meta-analyses by the group. According to these, statin treatment causes a small increase (about 1%) in reports of mild muscle pain in the first year and increases the risk of a new type 2 diabetes diagnosis by 10–12%. Researchers emphasize that this mainly concerns people who were already in a pre-diabetic state, and that the benefit of statins in preventing heart attacks and strokes significantly outweighs these risks.

Significance of the Data

This new study proves that the vast majority of side effects attributed to statins are not related to the drugs themselves, and the actual risk is extremely low. For instance, the annual increase in patients with abnormal liver tests was less than 1.6% compared to the placebo group. For edema, this figure was 0.07%, and for changes in urine composition, it was 0.03%.

The authors acknowledge the study's limitations: findings are based on physician reports rather than raw blood test data, which may lead to some underestimation. Nevertheless, researchers call for an update to drug leaflets. The fact that current leaflets are loaded with "unnecessary" side effects deters patients from vital therapy. In their opinion, information should reflect only proven causal links, allowing doctors and patients to make informed decisions based on facts rather than fear.
