Prof. Nir Barzilai, one of the world's leading researchers on aging and head of the Institute for Aging Research at Albert Einstein College of Medicine, has dedicated his career to understanding why some people live exceptionally long, healthy lives. His fascination with longevity began in his youth, observing his grandfather, and later deepened when he studied centenarians. Barzilai's research focuses on the biology of aging, suggesting that aging itself drives age-related diseases rather than being merely a byproduct of them.
The Biology of Aging and Centenarians
Barzilai and his team have studied a cohort of 850 centenarians, predominantly Ashkenazi Jews, who possess unique genetic traits that delay aging. Unlike the general population, these individuals experience almost no chronic illnesses throughout their long lives, with morbidity compressed only into their final days. Interestingly, their lifestyle habits often mirror those of the general public, indicating that genetics play a significant protective role against the biological mechanisms of aging.
Potential Therapeutics and Metformin
Among the various interventions being studied, the diabetes drug metformin has gained significant attention as a potential longevity-promoting agent. Clinical observations suggest that metformin users outlive non-diabetics on average, showing enhanced immune function and reduced systemic inflammation. Barzilai has been working to spearhead a large-scale clinical trial to definitively test metformin's anti-aging efficacy, navigating challenges related to funding and regulatory recognition of aging as a treatable condition.
Biomarkers and the Future of Geroscience
Developing anti-aging therapies requires reliable biological clocks to measure the rate of aging without waiting decades for clinical outcomes. Barzilai received substantial backing from ARPA-H to develop a comprehensive biomarker panel. While cautioning against unverified supplements and commercial pseudoscientific trends, Barzilai remains optimistic that ongoing research in geroscience will eventually allow humanity to extend both healthspan and lifespan significantly.
