The family of 42-year-old Nura Abu Adara, who died following a cesarean section, claims that medical staff missed critical time. Relatives state she spent four hours in the emergency room with severe pain before being transferred to the maternity ward, where a liver rupture was finally identified.

Four patients admitted for routine joint replacement surgeries at Ascension Saint Thomas Hospital Midtown in Nashville, Tennessee, suffered severe complications after receiving the wrong medication. Instead of the local anesthetic bupivacaine, they were mistakenly injected with potassium phosphate. At least one patient has been left without sensation in most of her body.
