For more than a decade, the reality television series The Biggest Loser captivated millions of viewers globally and in Israel, showcasing dramatic weight-loss journeys heralded as inspiring triumphs. Severely overweight contestants shed dozens of kilograms on camera, earning rapturous applause from audiences and producers alike.

Yet behind the television promise of transitioning to a healthy lifestyle lay a complex physiological reality that recently underwent its first-of-its-kind scientific examination. Researchers from the National Institutes of Health (NIH) in the United States decided to thoroughly examine the participants of the eighth season of the American version, broadcast on the NBC network.

Scientific findings on metabolic slowdown

The comprehensive clinical study, whose findings were published in the scientific journal Obesity in 2016, followed 14 of the 16 contestants from that season over six consecutive years. Simultaneously, an extensive investigation by The New York Times revealed to the general public the disturbing medical findings emerging from the prolonged scientific monitoring.

Precise laboratory tests at the NIH revealed a severe physiological phenomenon defined as persistent metabolic adaptation. In the scientific article, the researchers explicitly stated that continuous metabolic adaptation persists over many years as a response to ongoing human efforts to reduce body weight—a slowdown that failed to recover.

This is frightening and amazing, I was simply left completely stunned.

At the end of the six-year follow-up, it was found that the contestants' baseline metabolic rates remained lower by about 500 calories per day compared to individuals with identical body structures. Lead researcher Dr. Kevin Hall expressed his astonishment regarding the findings during an interview with The New York Times.

The heavy price of extreme routines

The consequences of the metabolic slowdown manifested directly on the scale: out of the 14 subjects, 13 regained most of the kilograms they had shed during the show's broadcast. Worse still, four of them weighed more at the end of the monitoring period than their original starting weight prior to the season's filming in the United States.

Alongside the metabolic damage, various investigations exposed the destructive physical routine conducted far from the camera lenses during filming. Contestants testified to a regimen that included six to eight hours of daily workouts, alongside extreme measures of intentional dehydration, exercising in trash bags, and prolonged stays in saunas.

Winner of the first US season, Ryan Benson, admitted that he fasted from water before weigh-ins and urinated blood due to severe dehydration. Additional contestants reported spitting into cups to shed mere grams, while medical examinations diagnosed cases of rhabdomyolysis, a life-threatening condition impairing kidney function.
